Follow up Information

Long term follow up
To make the information as complete as possible, your family doctor (GP) may be asked for a yearly follow up status related to your health and the breast cancer (if you have been discharged from your breast cancer specialists care). This will enable long term evaluation of the kind of breast cancer you had and the success of the treatments prescribed.

If you are a register participant and move away from the Waikato district you may submit this information by printing and completing the follow up form and return to us using one of the options below:-
